Complete the steps below. Once you have finished steps and pass the Bausch & Lomb VST Certification Questions (If you are already B&L VST certified we will
need your certification number on file), you will receive an email with instructions for downloading and installing the software, plus you will receive your account number.
- Click HERE to download, complete and fax the Online Direct Bill form to open your account.
- Click HERE to complete and pass the required Bausch & Lomb VST Certification Questions (takes approximately 10 to 30 minutes). You will receive an email indicating your results. (If you are already B&L VST certified we will need your certification number on file)
